This contact is only signed after the initial assessment session, and after an agreement has been reached between us to work together.

Your Counselling Agreement

 

This is an agreement between …………………………………. and Psychotherapist Katie McDonell.

We have agreed that sessions will take place on……………………………………(day) of each week, at……………………………….(time), beginning on……………………………………………..(date)

 

The fee for each session will be £45, for the maximum of 12 months. If you wish to continue with therapy after this time, we will review the work and the ongoing cost. The fee is payable weekly by cash or BACS only. Sessions will last for 50minutes and will usually be at the same time and place each week. Telephone contact between sessions (to cancel or rearrange only), will be between 9am to 6pm, Monday to Fridays. Voicemail or text messages can be left, I will reply to confirm I have received them between these hours. I will not be able to respond during vacation times.

I, the client, understand that;

·        If I cancel with a week or more notice I will not be charged.

·        If I (client) cancel with less than one weeks’ notice but more than 24hr hours – the cancellation charge is £10.

·        If I (client) cancel with 24 hours’ notice or less – I will be charged the full amount that I would normally pay.

If you miss two counselling sessions or fail to pay for two counselling sessions without contacting me to discuss this, I will write to you ending your counselling contract with me.

Holidays.

I will give you four weeks’ notice of any holidays I intend to take, and I would be grateful if you could give me two weeks’ notice of any holidays you intend to take.

Confidentiality.

Our work together is confidential, however there are some exceptions to this. As a requirement of ethical practice, I have a senior supervisor as do all reputable counsellors/psychotherapists, my supervisor is also bound by the standards of confidentiality. The only times where confidentiality would be broken is if I consider you to be in danger of seriously harming yourself our others, or where I would be required to do so by a court of law. I would make every effort to speak to you directly about this wherever possible.

Storage and Data Protection.

I am compliant with UKCP and BACP policies and procedures with regards to Psychotherapeutic practice. I am a registered data protection officer in compliance with I.C.O regulations for safe secure storage of records. You have a right to access the information that is held about you.
